Use this form to request Sacrament Certificates for Baptism, Confirmation, Holy Communion, Confirmation, and/or Marriage. Normal processing time for a certificate is 5 business days.

NOTE: If you are requesting any baptismal certificate in order to get married in another Catholic Church, certificates can be issued no earlier than 6 months before the wedding date to be valid.
